Low Back Pain - Abs or Back Strengthening?

Dear Physios,

People often ask that what would be a good exercise regimen for low back pain. Abs or Back Strengthening?

As what I came 2 know till now is that in case PIVD or IVDP, we cannot directly comment about exercise program without assessing the patient, and without correlating it with MRI reports……….coz every case is different from the other!

But then also, if the pain has reduced with modalities and manipulation,

Then the 2nd most step required is to start with static exercises, reason to maintain the tonicity of muscle, I.e. we should start with

- breathing 4 diaphragm (roof of core muscle)

-static abdominals(anterior wall)

-static back (posterior wall)

-and pelvic floor muscle (floor of core muscle)

This could b instrumental in preventing any future complication, and limits the relapse, when the affect of modalities subsides……..

After that, in the 3rd phase of treatment-we have to go 4 back strengthening, because the prolepses of the disc occurs mainly posterolaterally ,or posterocentrally, so we can prevent that by strengthening of back, but side by side we have 2 b continued with abdominal stats, so that they may not become weaker……….and also have 2 check that there must b not any facetopathy, because in that case back exercises contraindicated….

Then in the 4th phase- we can start with mild strengthening of abdominals also, when the back ms have been strengthened already but in the ratio of 3:1 4 back: abdominals.

Also when talking about PIVD, THE VERY FRST THING WHT ANY OF THE HEALTH PROFESSIONAL SHOULS DO IS TO TELL ABOUT THE PRECAUTIONS IN ADVICES,

AND IN THAT we say don't stop at all…………….

Although patient may feel a bit relax, but this will aggravate the condition………..so STOP STOOPING………….
